Equipment Hire Agreement
The terms below apply to the hire of a Healing PEMF & infrared mat from Longevity Lab. Please read them before booking your hire.
1. Parties & equipment
This agreement is between Longevity Lab (Hobart, Tasmania) — the Provider — and the person hiring the equipment — the Hirer. The hired equipment comprises a PEMF & infrared therapy mat, its controller, and a padded carry bag.
2. Hire period
Equipment is hired on a weekly basis from the agreed start date. The hire can be extended in one-week increments by giving at least two business days' notice, subject to availability. Early termination requires two business days' notice; any prepaid, unused whole weeks are refundable less a small administration fee.
3. Payment & deposit
The weekly hire fee and a refundable security deposit are payable in advance. Payment can be made by bank transfer or by Mastercard/Visa. Account and payment details are provided on your invoice at the time of booking. The deposit is refunded after the equipment is returned in good working order.
4. Care & usage
The Hirer agrees to use the equipment only as directed and in accordance with the supplied instructions, to keep it clean and dry, and to store it safely. The mat must not be folded sharply, submerged, or used by anyone the instructions advise against.
5. Liability & damage
The Hirer is responsible for the equipment while it is in their care. Any loss, theft, or damage beyond normal wear should be reported to Longevity Lab promptly. Repair or replacement costs for damage caused by misuse may be deducted from the deposit or invoiced separately.
6. Return process
At the end of the hire period the equipment must be returned, clean and complete, to Longevity Lab's Hobart clinic (address confirmed at booking) by the agreed date. Late returns may incur an additional daily fee until the equipment is received.
7. Health disclaimer
Healing PEMF & infrared mats are designed to support general wellness, recovery and comfort. They are not intended to diagnose, treat, cure or prevent any disease. If you are pregnant, have an implanted electronic device (such as a pacemaker), or have a medical condition, please consult your health practitioner before use.
8. Confidentiality
Any personal information you provide is handled in accordance with our privacy practices and used only to administer your hire and provide support.
Acknowledgement
By hiring the equipment, the Hirer confirms they have read, understood and agree to the terms above.
